Here are some pertinent resources and forms you may need to complete once you have initiated contact with the Short-Term Mission team, and completed the Initial Questionnaire. Have your driver's license and your passport handy, as you will need this information. This secure online application is designed to time out after 40 minutes, although you will have the opportunity to add time. If it does time out, your application will not be saved and you will need to start over. Please have your credit card handy, as you will be asked for it so that you can be charged for your background check.
